2 After Treatment 6-25-’94
A case of mild Class II malocclusion with a large overjet (9.5 mm).
The teeth are small, and the dental arch is spaced. In terms of skeletal structure, there is no anteroposterior discrepancy between the maxilla and mandible; this is a dental overjet. Early treatment was initiated with the goal of retracting the front teeth through short-term use of headgear, combined with correction of the molar relationship; however, due to insufficient patient cooperation, the first and second phases of treatment overlapped, resulting in a dynamic treatment period of more than four years. This is a case in which it appears that it would have been better to delay the start of treatment and wait until the permanent dentition was fully developed before proceeding.

There is a significant difference in shape between when the lips are relaxed and when they are closed, indicating that the harmony of the muscles surrounding the oral cavity is influenced by the position and shape of the hard tissues (jaws and teeth) (3). Localized orthodontic treatment was performed to retract the maxillary anterior teeth (4). Although the anterior teeth were retracted, the patient had entered the permanent dentition stage (5). Therefore, treatment was continued using full braces (6). Post-treatment lip shape (7).

On the cephalometric X-ray, the distance to the Nasion-A point is slightly short, and the mandibular ramus is also noticeably short; other than this and the fact that the anterior teeth are tilted forward, there are no other significant features. Although the mandible is slightly retrognathic, because the maxilla itself is positioned slightly higher, the mandible is not displaced anteroposteriorly relative to the maxilla (16). In such cases, excessive use of headgear is likely to cause the mandible to rotate clockwise.
